Statement of Credit:
In order to receive a statement of credit, you must complete the session, receive a passing score of 80% or better on the post-assessment, complete the evaluation form, and complete the online request process through My Transcript. Participants will be able to download a statement of credit upon successful submission of the credit request.To access My Transcript, visit www.diaglobal.org, select “Sign in” and you will be prompted for your user ID and password (if you do not know your username and password, choose Forgot Your User ID/Password). Find EVENTS, select “Continuing Education”. Select the blue “My Transcript” button followed by “Credit Request”.
