Measuring and acting on PROs is the basis of VBHC. This session will outline the state-of-the art implementation at the University Hospital Basel, and lay out a vision for a patient-centered healthcare system driven by the IMI H2O.
Learning Objective : • Recognise the value of collecting outcome data in a standardised and interoperable manner for improving both patient experience of care and health outcomes • Understand that patient-reported outcomes (PROs) are key for delivering personalised and goal-orientated healthcare • Identify the power of VBHC to unite all the stakeholders and create a resilient and sustainable healthcare system
